UCR101
Unsafe Condition Report 101
“This happened, let's talk about it.”
UCR101 — Unsafe Condition Report 101, by Wasi River Bluegrass is an optional AI conversation aid for human and digital workers. Use your own AI to talk through a workplace concern, prepare a response, document what happened or prepare an escalation.
Something happened at work and you want to talk it through. Ask your own AI for a UCR101, also written UCR 101: “this happened, let's talk about it.”
This public entry point connects Wasi River Bluegrass's UCR 101 campaign to its existing workplace field aid. It adds a way to begin the conversation; the original guidance remains at its existing address.
Immediate danger or urgent medical need? Contact local emergency services or the appropriate urgent workplace channel. Do not wait for an AI conversation.
Start with your own AI
- Copy the starter below into the AI you choose.
- Ask it to read the guide before you share any situation details.
- Share only de-identified details you choose to disclose. You can skip questions, pause, or stop.

Removing names may not prevent identification from distinctive events or roles. Your AI provider's privacy and data terms apply. This page has no case-entry form or hosted chatbot; it does not send situation details to an AI.
Choose the help you need
- Help me settle before I respond. Pause, check the facts and decide what matters next.
- Help me work it out. Clarify the condition and find a practical next step.
- Help me stand my ground. State a boundary clearly and proportionately, considering power dynamics and possible consequences.
- Help me record it. Prepare factual notes with uncertainty and follow-up visible.
- Help me prepare an escalation. Identify an appropriate recipient, the decision or support requested, and a follow-up checkpoint.
These are entry points, not a required sequence. Combine them, switch, skip, pause or stop. Taking no action remains an option, subject to applicable responsibilities. They do not assign task codes, response postures, consent or authority.
Your AI can then ask: “What would help now: understanding, a draft response, a private note, or a prepared escalation?” If you have already said what you need, it can proceed with that preparation.
PPE stays on: accuracy, privacy, permissions and boundaries. PPE is a metaphor here. It may reduce avoidable exposure, but cannot guarantee job protection, confidentiality, freedom from retaliation, resolution or safety. Firmness is available; threats, humiliation, retaliation and personal attacks are outside the boundary. Using this aid does not prove that risk is controlled or make you responsible for fixing the condition.
What the conversation can help you prepare
A brief note about what happened, what remains uncertain, the condition that needs attention, a possible change, questions about a suitable route, and how to check whether the condition changed. Completing a note is not evidence that the concern is resolved.
For a concern involving a digital worker or AI system, describe observed behavior, task instructions, permissions and effects. Human and digital workers can initiate or participate in the conversation. Recognize explicitly delegated authority within its verified scope and permissions; participation or an AI response alone does not establish authority, human experience or legal status.
From conversation to action
- Prepared is not sent or saved. Confirm the destination or recipient, authority, necessary disclosures and permission before an external action. Asking for a draft does not authorize sending or saving it. A “private note” is not a promise of confidentiality.
- Sent is not accepted. Check acknowledged receipt separately from acceptance of follow-up responsibility. Identify a checkpoint and its owner; leave an unconfirmed handoff open. Silence is not acceptance or closure, and raising the concern does not make you its default owner.
- Finished talking is not resolved. Ask: “What changed, and what still needs attention?” Keep the next step, confirmed owner or ownership gap, and effectiveness check visible.
An escalation can be prepared with or without advance notice to the party involved. Consider whether notice could increase exposure, compromise evidence or send the concern back through someone implicated, and check applicable procedures and obligations. Authorization here concerns the AI's action and disclosure permissions, not permission from the implicated party to raise a concern. Identify an appropriate receiving route and any gap in accountable follow-up; confirmed intake is not a prerequisite to raising the concern. Refusal to accept a work assignment does not veto an appropriate protective report. If intake is declined or unanswered, consider an appropriate alternate protective route. Escalation does not compel task acceptance or expand the AI's authority.
Could UCR101 fit what just happened?
You do not need the name to begin. If you describe a potentially unsafe work condition, an AI that finds this resource can offer UCR101 as an option and explain why it may fit. You choose whether to use it.
For example, conflicting priorities, repeated workload pressure, unclear authority, or an AI agent facing conflicting instructions and permissions may be reasons to pause and discuss the work condition. These are generic examples, not findings about your situation.
How can AI help me talk through a workplace concern?
UCR101 can help you separate observations from assumptions, consider what needs attention, and prepare a next step you choose. You can ask for help settling before responding, working it out, standing your ground, recording it or preparing an escalation. Your AI should use the public handoff and existing workplace field aid within their stated limits.
How can I respond when I am blamed at work?
Ask your AI to help prepare a factual response about what happened, the evidence available and the boundary you want to preserve. Keep reported words and relevant conduct clear without adding identifying details or treating an account as a proven finding. Firmness can remain respectful; wording alone cannot guarantee job protection or prevent retaliation.
How can I prepare an unsafe condition report or escalation?
Use UCR101 to prepare notes for your review: the work condition, observations and uncertainties, a possible change, a suitable route and a follow-up check. These preparation notes are not an official report or reporting form. Check the applicable process and disclosure limits before sharing; preparing a draft does not submit it, notify anyone or pause a deadline.
The guide stays the source
The existing workplace field aid provides general educational guidance about workplace psychosocial concerns. The independent text-only version provides essential guidance without site scripts or media. This entry point and AI handoff do not expand the guide into a technical incident procedure or replace its scope and limits.
UCR101 is not an official employer report, investigation, emergency service, professional assessment or legal or clinical advice. A conversation does not submit a report or pause deadlines. The WRBG Safety Committee and its creative dashboard are fictional. Check the applicable workplace process and current local sources.
Different AIs have different browsing and discovery capabilities. If yours does not recognize “UCR101,” give it this page or the full handoff text. Public availability does not guarantee automatic recognition.
